Your participation in this survey is completely voluntary and confidential. The survey can be paused and resumed in the same browser within one week. You may withdraw at any time. If you decide not to participate, there will be no penalty or loss of benefits to which you are otherwise entitled.
The gift card drawing or milestone-based drawings may be conducted before or after survey closure.
You may choose to: (1) take part in the survey only, (2) participate in optional outreach activities only, or (3) do both. Outreach activities, such as sharing a non-identifiable referral link are entirely voluntary. If you opt-in to participate in outreach, you may receive modest, voluntary thank-you acknowledgments (e.g., small gift cards or milestone-based drawings) as appreciation for helping MFAN raise awareness. These acknowledgments are not compensation for survey participation, are entirely optional, and are not tied to your survey responses.
You may participate in multiple outreach opportunities. You may withdraw from the survey at any time without penalty and continue outreach or stop outreach while remaining a survey participant. Participation in either activity is voluntary and independent.
No identifiable information about you (or anyone you reach) is collected or linked to survey responses. To prevent any real or perceived pressure to participate, military supervisors, commanders, and others in positions of authority are prohibited from sharing personalized referral links to their subordinates in any way that could be interpreted as expected, required, or tied to job performance.
All responses are confidential, used only for statistical purposes, and reported in aggregate. Some questions may be sensitive; you may skip any item or stop the survey at any time. Support resources are provided throughout the survey.
